Hospital substitution is not a new concept and has been a feature of our health system for many years. With increased demand for expensive hospital resources OMC recognises that not all care needs to take place in a hospital environment with evidence suggesting that care in the home (for suitable patients) can in fact lead to faster recovery and better outcomes.

Is hospital substitution for me? 

Your hospital doctor will recommend this service if they feel your condition can be managed safely at home.

The following factors are considered to determine suitability for care in a home environment:

  • Is your treatment suitable for home care?
  • Are you clinically stable?
  • Do you have a support person to look out for you?
  • Do you have a phone so you can contact someone if you have any problems?
  • Do you want to be treated at home?

If your doctor believes you are suitable for our hospital substitution program an assessment appointment will be made with our medical specialist.

How does this work.

  1. All eligible inpatients are assessed in hospital for suitability for our hospital substitution program.
  2. Once approved by the Medical Director, your consent to treatment is obtained. A care plan is developed in partnership with you and or your carer.
  3. You will be provided with an expected length of care and treatment and advice on any out of pocket expenses.
  4. Your care will be routinely monitored either remotely or with a daily visit by one of the registered nurses.
  5. You can access help or advice 24 hours a day through a direct contact number.
  6. When your treatment is completed, you will be assessed for discharge back to your General Practitioner.
  7. In the event of any medical emergency, Onsite Medical Care will arrange for an immediate assessment and transfer to either a public or private hospital. This will depend on the level of the emergency.

Our commitment to
safety and quality

Onsite Medical Care is committed to external review and is participating in the ACHS EQuIP 6 accreditation program, and is currently accredited by ACHS.

To support our clinical governance accountabilities OCM has a Clinical Governance and Medical Advisory Committee. These committees oversee our safety and quality performance outcomes.

Onsite Medical Care is also committed to ensuring consumers are engaged in our safety and quality programs. All patient-related information is reviewed by consumers prior to publishing. We will also be undertaking surveys and focus groups so consumers can give us feedback on how well we are meeting their needs.

All clinical staff is required to undertake mandatory training and demonstrate compliance.

For referrers

If you are interested in our service model or wish to discuss a potential patient transfer, please contact us. Our medical practitioners are all credentialed to provide services. Our registered nurses are experienced in delivering hospital substitution nursing care.

We offer consultant physician services supported by our trained registered nurses.

  • Cellulitis – minor and major complexity
  • Minor urology infections
  • Postoperative wound management
  • Congestive cardiac failure
  • Venous thrombosis
  • Pneumonia
  • IV hydration therapy
  • Allied Health – If required, referrals can be made for physiotherapy support in the home.
